A signaling system can be perfectly designed and still fail in the field if it's never properly tested and commissioned — which is why T&C sits at the safety-critical junction between installation and operations, not as an afterthought before handover. FAT, SIT, and SAT exist precisely because a component that passes in a factory doesn't automatically behave correctly once it's integrated with ETCS, CBTC, or electronic interlocking on site.
